Forms you submit to USDA
Mirrored copies of the official USDA forms. You file them with your local USDA Service Center, not with us. They ask for tax ID details; those go only to USDA.
NRCS-CPA-1200 Conservation Program Application
The application itself. Write "Regenerative Pilot Program" under Assistance requested.
AD-2047 Customer Data Worksheet
Sets up your USDA customer record. Needed for a farm number.
CCC-941 Adjusted Gross Income certification
Payment eligibility form.

CCC-902I Farm Operating Plan (individual)
Usually completed with your FSA office. Use the entity version if your farm record is under a business.

USDA's Natural Resources Conservation Service runs the Regenerative Pilot Program and makes all funding decisions. Mirrored forms are copies of the official USDA versions on the date shown; if USDA has issued a newer version, file that one. "Regenerative" has no federal legal definition.
